The Secretary heals 1 Sanity per check-in for 120 coins, and check-ins are faster and safer than treatments, so the cheaper class usually out-heals the expensive one. Meanwhile the 500-coin Psychologist doubles every Sanity gain you make and starts at 100% Sanity, dominating the sustain niche the Doctor wants to occupy.

Buy the Doctor if you main the treatment role and want your healing to feed itself, or if you simply like the deer in the coat. But if you are climbing the class ladder efficiently, most players should either stop at the Secretary or save the 900 coins toward the Surgeon, which pays you Sanity and speed for the exact same activity.
