The Doctor Running the Night Shift

Dr. Harlow is a semi-bright orange muntjac deer with grey antlers, a head mirror, a stethoscope, and a bright blue surgical mask - redesigned in the July 10, 2026 “Harlow’s Favorite Update.” He is the head doctor of the hospital, your mentor, and your direct supervisor; the wiki does not name a medical specialty beyond that. He carries a black suitcase stamped “TOP SECRET,” and unlike every other main character, his anomaly status is listed as “Unknown.” Notably, the game’s creator refers to Harlow with both he and she pronouns.

Shift-by-Shift Appearances

Harlow shows up at the reception window at the end of the early shifts, unlocking the game piece by piece:

• After Shift 1: advice, plus the shutters and security cameras.
• Shift 2: the Supplies Shop and the Emergency Wing (Rooms 6-8).
• Shift 3: the taser.
• Shift 4: he warns you an anomaly attack has hit the town and an ambulance is coming - then runs away instead of helping. Survive the ambulance event and he returns with congratulations and a bonus.
• Shift 6: he warns that losing three patients ends the run, and tells you your sanity is important - “Don’t DIE on me.”

After Shift 7 he stops coming to the window, but he stays in the lobby (and sometimes wanders to the Supplies Shop), where you can walk up and talk to him between shifts. He also hands out a performance grade report after shifts and brings bonuses during emergencies - the first completed ambulance event pays $150.

Mint Tea and the Lore Menu

Harlow’s favorite drink is Mint Tea ($100 in the Supplies Shop). Offer it to him in the lobby after his initial dialogue and he accepts, then answers one lore question per shift. The menu covers Ratthew, Ron, Barney, Liz, the Animal Corporation, and Oingit, the previous hospital manager. Ask again in the same shift and all you get is “sigh” or ”…”. These answers hold real revelations: “From Accounting” is Ron’s actual last name, Ratthew is “good intentioned,” Liz should have stayed silent, and there were many managers before Oingit and you - all of whom presumably disappeared.

His Office and What He’s Hiding

Harlow’s office is the room labeled “NO ACCESS” near the Supplies Shop - it is where he flees when the ambulance event begins. The wiki records several details tied to it: a locked book by hospital founder John A. Nomaly titled “On The Discovery and Nature of the Anomaly Gene,” a whiteboard DNA drawing that appears in Ratthew’s secret vent tape, and an easter-egg deer from 99 Nights in the Forest. Ron from Accounting enters the office to complain about Ratthew stealing paperwork. Other secret interactions: he periodically steps outside to meet a whispering Animal Corporation employee in a hazmat suit, returning with boxes marked “confidential,” and the key Ratthew steals - the one that opens the Vault - originally belonged to Harlow and can even be handed back to him.

His lobby monologue is the darkest lore in the game: the Corporation “saved” him - “or at least what was left of her” - which is why he serves them, and he believes that if their research succeeds there will be no more patients, no more endless shifts, no more trouble at all. What that research costs is a question he never answers.
