Animal Hospital Sanity Guide: Every Gain and Drain

Sanity is Animal Hospital’s health system. Every player starts a run with 61-65% base sanity (some classes add more), and hitting 0% kills you. Nothing else damages you directly — every monster, event, and mistake in the game is expressed as a sanity number. Learn the numbers and you control the game.

Gaining sanity

Losing sanity

Passive and self-inflicted:

  • Job stress: -1 every 50 seconds, always. From 100%, stress alone kills in 1 hour, 23 minutes, 20 seconds.
  • Tasing a normal patient: -15
  • Getting tased by a teammate: -2 for both of you
  • Shooting a teammate with the Gun: -6
  • Looking at a cursed photo: -10 (inspect polaroids on the desk instead of holding them)

Events:

  • Standing near fire: -1 to -2 per second (level 3 Firefighters are immune)
  • Extinguishing ritual candles without Eye Drops, IV Drops, or Coffee equipped: -1 to -3 per candle

What low sanity looks like

The game warns you before you die. Below 35%, the music slows down and the environment desaturates. Below 30%, the posters in Rooms 1, 2, and 7 begin to change, growing more disturbing the lower you go. Both effects reverse once you recover above the thresholds. Treat the desaturation as your cue to stop treating patients and go find coffee.

One class-specific wrinkle: with the Psychologist class equipped, all sanity changes — positive and negative — are doubled. Chocolate heals 120, but a Camera Monster jumpscare costs 40. High risk, high throughput.

Managing the economy

The math is forgiving if you respect it. Job stress costs 1.2 sanity per minute; one free coffee every few minutes more than covers it. What actually kills players are burst losses — a Stalker glance into a Head Banger dismissal is 30 sanity in seconds. So:

  1. Grab coffee the moment each shift starts so the machine brews the next cup during the shift.
  2. Keep a chocolate banked for after an enemy encounter, not before.
  3. Never tase on suspicion. A wrong tase (-15) costs as much as half a coffee cycle restores; confirm anomalies with the photo camera or scanner first, as covered in the how to play guide.
  4. Pick synergy classes for long runs. Secretary and Doctor turn your normal workflow into passive regen — more on that in the patients guide.
