The Clinic Cast

Not everyone who knocks on the check-in window is a patient. Recurring characters visit across shifts, and how you treat them decides whether they come back: some hand you items, some open questlines, and one of them hands you the key to the game’s biggest secret. Turn a character away — or ignore what they ask for — and they may not return for the rest of the run.

Staff NPCs vs Story Visitors

Two kinds of non-patients show up. Staff — Dr. Harlow, the Shopkeeper Nurse who runs the supplies shop, the Hazmat Team during slime cleanup — belong to the hospital and appear on schedule. Story visitors arrive at the check-in window like patients and force a decision: admit, reject, or interact. The wiki documents six recurring visitors, and several of them stop returning if turned down.

One rule saves a lot of second-guessing: staring does not make someone an anomaly. Ratthew stares at you through the window and is explicitly not an anomaly; the anomaly tells that actually matter are facial distortions, photo defects and camera behavior.

Why Characters Matter for Lore

Dialogue across encounters — especially Ron from Accounting’s and Dr. Harlow’s — reveals that anomalies are infections caused by Animal Corporation experiments on hospital patients, transformed gradually over repeated visits. Liz, a news reporter, was fired for getting too close to that truth. The vault ties the threads together with a photo of Rattina and a note about “the Shadow.” If you care about the story, the character encounters are the story — the ending is told through them, not through a final shift.

Encounter Tips

  • Keep a spare coffee for Barney; a fed Barney is a calm Barney.
  • Take Ratthew’s key even if you plan to hand it to Dr. Harlow — the choice itself is content.
  • Hire Sam after a Slime Shift if you can spare $250; three shifts of slime, Hider and fire cleanup is worth it in co-op chaos.
  • Meeting needs costs sanity or money now, but pays back in perks and story later.
