Animal Hospital Enemies: All Monsters & What They Do

The Roster

Eight enemies debuted at the game’s May 10, 2026 release — Bed Monster, Stalker, Mass of Eyes, Hiders, Mimic, Head Banger, Tentacle and Camera Monster — and updates have kept adding to the ward since, most notably Slimebert and the Tiny Anomaly in the July 25 Hazmat update. Each one attacks sanity differently, so each has its own counter.

Supporting Cast

  • Hiders — blend into walls, never alone; getting close triggers a chase and a -10 sanity hit.
  • Head Banger — hollow black eyes, open frown; calms down if given coffee, which makes it one of the few enemies you can bribe.
  • Tentacle — grows out of an admitted anomaly mid-treatment; a room 8 special.
  • Camera Monster — the reason checking the monitor is not always free.
  • Slimebert — roams the wings during Slime Shifts, eating anyone in its path: -20 sanity on grab plus -3 per second while digested. Patients it hits faint; anomalies it hits turn into Mimics.
  • Tiny Anomaly — a tiny pink bunny with three red eyes and a squeaky voice; harmless-looking until checked in.
  • Lobby Bunny and Lobby Cat — waiting-area creatures with their own rules; the lobby is not a safe room.

Anomaly or Enemy?

The game splits the danger in two. Anomalies are disguised patients you can stop at the window with the three-check routine. Enemies are what you deal with after the disguise drops — or creatures like the Stalker and Ghost that never bothered with a disguise at all. Rejecting well reduces enemy encounters; it never eliminates them.

Universal Survival Rules

  1. Guard sanity like ammo. Coffee (+15), chocolate (+60) and mint tea (+15-30) are your heals — the sanity guide has the full economy.
  2. The taser is defense and profit. Tasing an anomaly pays +2 sanity; Security starts every run with an X-Taser.
  3. The gun ends arguments. The Secret Agent’s 20-use gun kills anomalies outright and, at level 3, pays sanity and cash per kill.
  4. Log everything. Each enemy encountered fills a Journal entry worth Animal Coins — even bad nights pay something.
  5. Emergencies stack. Fires, rituals and ambulance waves interact with enemies; the events guide covers triage order.
