All 11 Animal Hospital Classes: Prices & Perks Compared
How Classes Work
Classes are player modifiers you buy in the lobby’s class shop — walk into the large yellow circle to the left of spawn, next to the ambulance. Each class changes your character model, shows its name above your head in the lobby, and grants a perk that supports one part of the night-shift loop: starting sanity, inventory space, consumables, or outright weapons.
Every class has three levels. Upgrading strengthens the level 1 perk or adds a new one — the exact effects per level are on each class page below.
Every Class at a Glance
| Class | Cost | Level 1 Perk |
|---|---|---|
| Intern | Free (default) | +10 starting sanity (15/20 at levels 2/3) |
| Nurse | 20 Animal Coins | +1 inventory slot per class level |
| Secretary | 120 Animal Coins | +1 sanity per patient checked in |
| Paramedic | 250 Animal Coins | Large Run Fast Cola, 6 uses |
| Psychologist | 500 Animal Coins | Doubles all sanity effects, starts at 100% |
| Doctor | 900 Animal Coins | +1 sanity per patient healed |
| Security | 1,250 Animal Coins | X-Taser with 5 uses |
| Surgeon | 2,500 Animal Coins | Sanity + speed boost per treated patient, $35 starter money |
| Head Nurse | Robux only | +3 inventory slots |
| Secret Agent | Robux only | Gun (20 uses) + scanner + 2 inventory slots |
| Firefighter | 1,100 Animal Coins | Self-recharging Special Fire Extinguisher (added July 25, 2026) |
For rankings rather than raw data, see the class tier list — it orders all 11 by run value and cost efficiency.
Which Class Should You Buy First?
- First 20 coins: Nurse. Inventory space helps every playstyle and the price is trivial.
- First real save-up: Secretary at 120 coins if you hold the check-in desk, or Psychologist at 500 if you can wait — its doubled sanity gains carry entire runs.
- Long-term coin goal: Surgeon for treat-tempo runs.
- If you spend Robux: Secret Agent is the strongest kit in the game — a 20-use gun plus a scanner trivializes anomaly handling.
Classes and Sanity
Sanity is the health bar, so class choice is mostly a sanity decision. High-starting-sanity classes per the community wiki: Intern, Secretary, Psychologist, Doctor and Surgeon. The Psychologist is the extreme case — it starts at 100% and doubles every gain and loss, with level 2-3 able to ignore negative effects.
Skins
Every class except Firefighter has at least one skin — purely cosmetic recolors and redesigns bought with Animal Coins in the same shop. Classes redesigned since launch also keep their pre-July 10 look as a “classic” skin.
